NOTES

Work crew should initial each task that they work on.

Supervisor should indicate the date as each task is completed.

QC person should fill in the report for each QC task. The QC tasks must be completed before subsequent tasks are started.

Work crew shold fill in the materials list for all items used.

Note any problems, deviations from procedures, suggestions, etc. in the space at the bottom and on the reverse side of pages.

NOTES:

5.3 The laser fiber bundles should go to the outside of PMT's 1&2 - except for the fibers going to PMT's 1,2,3 - fixed by RJM

The L bundle to PMT10 is on the wrong side of R bundle to PMT12 (fixed, RJM)

The Laser fibers to PMT 42,44 were interchanged, fixed, RJM

Laser fiber to PMT7 should be on the outside of E1 bundle - fixed, RJM

Laser fiber to PMT13 is wrapped around the rail - too much stress - fixed, RJM

5.4 A15-PMT30 - The left bundle is misrouted to PMT34

B14- PMT34 - Right bundle is misrouted ot PMT30

B11 - PMT10 - Left bundle is entangled with the R bundle to A13-PMT12 - should go under it as per model

Laser fibers should be routed outside of PMT1 - see model

None of the bundles have been marked before inserting into the aspirin tubes
