Work crew should initial each task that they work on.

Supervisor should indicate the date as each task is completed.

QC person should fill in the report for each QC task. The QC tasks must be completed before subsequent tasks are started.

Work crew shold fill in the materials list for all items used.

Note any problems, deviations from procedures, suggestions, etc. in the space at the bottom and on the reverse side of pages.

NOTES:

7.2,3: Examination of the cut-off ends of aspirin tubes revealed 2 on the even side and one on the odd side with voids in the epoxy. One of the cut-off on the odd side showed no fibers. Aspirin tubes will be numbered on the bottom from now on to help identify the problems.

    1. Inspection with the video camera showed:
PMT13 - These fibers were not at the end of the aspirin tube. These fibers were long enough sothat we could just cut them off at the entrance to the aspirin tube and then insert them into a new aspirin tube. The fiber bundle was glued into this aspirin tube and then repolished.

PMT7 - A void on the bottom of the aspirin tube was filled with epoxy and then repolished after removing the washer.

PMT24 - - One sub-bundle of fibers not at the end of the aspirin tube - this was fixed by breaking off the aspirin tube and replacing it with one that was drilled out so that the fibers could be inserted to the bottom of it after cutting off about 1 cm from the glued bundle. The fiber bundle was glued into this aspirin tube and then repolished.

In addition we realized that we had forgotten to put profiles on the extra tiles on the exterior of the ITC. These were added and the fibers were glued into these aspiring tubes after pulling out the Teflon rods.
